Following the events after the US ending, Sarah has escaped from the cave and has found aid at a nearby hospital. Now, mentally scarred and unable to speak, she helps lead a search and rescue team back to the cave in hopes of finding survivors.

Things are moving along for the unecessary sequel. It appears that the actresses that played the characters of "Sarah" and "Juno" will return, and the premise can be read above. At the Film 4 FrightFest in Britain, Neil Marshall revealed that editor Jon Harris will make his directorial debut with the film. Jon has served as editor for various films, including the first Descent, and Marshall reportedly said there was no better man for the job. The script is currently still being worked on. Shock spoke with Descent creator Neil Marshall recently, where he revealed that the silly title above indeed is the actual title (for now anyway). He also went on to say how he won't write or direct the sequel, but he will be involved in the story-making process, as he and the producers have thrown some ideas around, one of which plans to bring back the lead from the first, "Sarah". Either way, the project is still in the very early stages and won't happen anytime soon.

Makes you think she went crazy and killed them all...and that there were actually no monsters.....very interpretive.
That's the UK ending you're talking about, the original ending. They changed the ending for the release in the states and the movie ended before she woke up back in the cave, leading the audience to believe she had escaped.
I personally think the UK version is better.
