The Pusher trilogy is a good set of films if you're looking for a gritty realistic approach to the underground. I personally like the second film the best of the three.

If you end up enjoying the Pusher films I'd also recommend the 1997 Russian film "Brother". I made a post about it a while back, but only Ashvamp seemed to notice:

Aside from that, one of my all-time favorite gangster films (aside from the obvious ones of course) is Takeshi Kitano's film, also named "Brother".

I've seen a lot of other good gangster films to come out of Japan, and some decent ones to come out of Korea, but I'm currently drawing a blank for them, plus, the ones from Japan are generally only available online anyway.
