Things That Do:
When children are murdered
When a person is cut in half (vertically or horizontally)
When zombies outnumber a small group and attack
Demonic possesion
When a slasher/killer/psycho keeps some momento from their victims.
(i.e. clothing, part of a body, picture)
Some forms of torture
Things That Don't:
Ghost stories
Killer dolls or any toys for that matter (like Chucky :die:)
Gun shot kills to the chest
Psycho families that attack unsuspecting teens(and manage to go vitually unnoticed by the aurhorities for years)
Underwater animal threats (with the exception of JAWS of course)
....more to come

Sudden noises or things jumping out and gore in general don't creep me out. But cannibalism or mutilation of a live person does definitely creep me out. I guess it's just that thought of being alive and in immense pain that really does it to me.

When puppies die I'm freaked out.
I agree about toys and dolls not being creepy at all. I'll also add mean kids (Daddy's Girl, The Good Son, ect).

I seem to freak out mostly when what I'm watching is real. Like Mondo. First time I watched Blood Of The Beasts was freaky.
Also, water. Not just water, but stormy water, or sometimes just if the water is the "second enemy". Deep Blue Sea even freaked me because it was stormy. Titanic too. Jaws is the leader here, both for scary fucking shark and the water. It's weird, I'm not scared of water, it's mostly the thought of how hopeless you'd feel trapped on a sinking boat or being in water in the middle of nowhere.
Don't:
The list is too long.

The only thing that gets me is when small children are harmed, I hate seeing that. Most recent one I saw was a scene in The Poughkeepsie Tapes that was very disturbing

Does:
Mob minds, if done right. Romero's NotLD. Not Stephen King's Cell.
Distorted perceptions. FearDotCom had some big issues, but the characters' inabilities to know reality from hallucination is the best immediate example.
Claustrophobia. Again, it has to be done correctly.
Does not: Kids in Peril.
It's every bit as much of a cheap ploy as some murderer stalking teenagers. Double suck if the kid shows psychic abilities.
Religious horror.
What kind of pussy is Satan if he can only deal with one little girl at a time? Then he beat off and threw up? Sounds like some toothless country & western fan after a night spent drinking at Midge's Catfish & Rodeo.
Yet Another Grudge.
One time some chick with long hair died and got sad because death sucks so she's killing everyone else and now everyone else is dying but first they had to open up the newspaper to the funny pages and look at the crossword and if the word "BLOOD" was filled in the crossword puzzle with red lettering then Chick With Long Hair was totally going to climb out of the sports page and when the cops find the body it has a french tickler in one hand and a jumbo child's crayon in the other hand. It's as though there's one script with a lot of blank lines and someone else is filling it Mad-Libs style.
Choose-Your-Own-Reality.
Was that duck really a duck? Was it Satan's duck? Maybe it was the angel Raphael in disguise. Someone else once had a theory that the duck was really a martian from the year 11,432. The clues go in all directions. That movie was sooooooooo deep!
Holy shit! They were dead the whole time!
You can tell this is coming when the rest of the movie features the most boring ghosts and/or haunted house put to film in years.
Not Really Supernatural
Some guy in a tie hears about dead people, meets some creepy people, performs some suspenseful breaking & entering, crawls in a dark place, and ultimately finds out the killer was really just the night janitor in a mask. Sounds like something that bored me when I was 9.

killers mocking their victims
helplessness
torture
child murder (not teen...but actual children)
teeth being knocked out/chipped out/ripped out
fingernails being torn off
suspense

everything else
mainly cheap scares (her boyfriend hops out a closet and the music climaxes...but it's just him handing her a beer)

Things that freak me out:
I agree with The Spine Splitter: fingernails getting ripped out...makes me cringe just thinking about it. Also, seeing someone getting sliced in certain places, like in Hostel, when the dude's Achilles tendon is sliced. It makes my stomach turn.
Also, due to a phobia, I can't do ventriloquist dummies. Cannot do it. I cry. Wax figures and some mannequins freak me out too, like in The Hills Have Eyes. The mannequins in the A-bomb test town freak me out more than the inbred monsters.
Great suspense is also good for a quick scare. I love it when you KNOW the killer/monster/ghost/whatever is going to pop out...you just know it, but then it doesn't happen right away. The intensity grows and, even though you know it will happen, it still scares the shit out of you when it finally does. I love that.
I can handle just about everything else.

Does:
* Innocents being killed (murder of children and animals)
* Finger/toe nails being ripped out, or the fingers and toes being crushed or broken
* Teeth being forcibly removed.
* Needles in the eyes (Audition really got me there)
* It being directed by Uwe Boll
Doesn't:
* Gore (The more the better)
* Torture
* ghosts with grudges
* Killer technology
* Most things...

Cell hasn't even come out yet. And what was wrong with the mob mentality in the book anyway :shrugs:.
As you kind of stated, there has been a book released of it. And Eli Roth does suck a little, assuming he is still attached. Cell itself just felt like it kept doubling back on whatever story it was trying to tell.

Example: The Shining (I'm just gonna bash your brains out) or anything Loomis says "I tried to burn him to hell but in my heart, I knew hell would not take him"

Things that Do:
The scariest thing for me is usually the unknown, sometimes not seeing the monster or bad guy (whatever you wanna call it) is a good thing. Seeing it is sometimes disappointing and takes the scariness out of it.
Also, when people are stuck in close, confined spaces it seems to creep me out. A good example is in "The Decent" when their climbing through those narrow tunnels.
Dont:

Maggots, something about Maggots! I can't stand Maggots, dunno why, but they're the one thing I can't handle. That scene in Feast where the guy had maggots in his eye, I gag just thinking about it.
Things that don't:
Not really much else bugs me. Spent 8+ years in the Army and saw enough gore, really sick stuff, that not much bugs me. Though people on fire does bother me a bit, but that's kinda a personal story.

another thing that bothers me is Dana Kimmel's monologue in F13 pt 3. Her delivery just irritates the hell out of me.
